If you qualify for a subsidy on your health insurance, you must annually complete the Adjusted Health Insurance Subsidy Form (DOC) and submit to HR. This is for employees with a household income below $45,000. The health subsidy forms need to be submitted by December 1, to be effective January 1, 2017.
